Get Ready
A little preparation goes a long way. Here's what to know before you arrive at our Summit office.
If you are receiving sedation or general anesthesia, you must have a responsible adult drive you home after your procedure. You cannot drive, operate machinery, or make important decisions for 24 hours following sedation.
Download and fill out the patient forms above. Bring them to your appointment to speed up check-in.
Have your dental and/or medical insurance card ready. We'll verify coverage before your procedure.
Bring a valid government-issued photo ID (driver's license, passport, or state ID).
Bring a current list of all medications and dosages, including over-the-counter supplements.
If referred by your dentist or physician, bring any X-rays or referral paperwork they provided.
This gives us time to review your forms, verify insurance, and make you comfortable before your appointment.
Financial Info
We work with most major insurance plans and offer flexible payment options to make your care affordable.
We accept most dental insurance plans including Delta Dental, Cigna, MetLife, Aetna, Guardian, and more. We'll verify your benefits before treatment.
Certain procedures may be covered under your medical insurance. We accept many medical plans and can help determine coverage.
No insurance? No problem. We offer multiple payment options and financing plans to fit your budget.
Your Comfort
We offer multiple levels of sedation so you can choose the comfort level that's right for you.
Also known as "laughing gas," nitrous oxide helps you relax during the procedure. You'll remain awake and responsive but feel calm and comfortable.
Moderate to deep sedation administered through an IV. You'll be deeply relaxed and may drift in and out of sleep during your procedure.
Full unconsciousness for patients who need extensive work, have severe anxiety, or prefer to be completely asleep during surgery.

A referral is not required for most services, but some insurance plans may require one. We're happy to check your benefits and let you know.
If you're receiving sedation or general anesthesia, do not eat or drink anything (including water) for at least 8 hours before your procedure. For nitrous oxide, a light meal beforehand is fine.
Recovery varies by procedure. Simple extractions may heal in a few days, while implant placement or bone grafting may take a few weeks. Dr. Kim will provide detailed post-op instructions tailored to your specific treatment.
